• Improve population health by strengthening the ability of primary care to operate collectively at neighbourhood scale, enabling earlier intervention, better coordination of care, and more consistent management of people with long-term and complex conditions.
• Champion the values of local partnerships, professionalism, and trust that underpin general practice.
• Create fair and transparent opportunities for all member practices delivering frontline care and their patient populations.
• Work collaboratively with PCNs, neighbourhoods, and system partners to design sustainable, integrated models of community healthcare.
• Coordinate and strengthen the role of Patient Participation Groups (PPGs) at neighbourhood level, creating a meaningful patient voice to inform service design and improvement.
• Invest in workforce, technology, and innovation to secure the future of primary care for the next generation of clinicians and patients alike.
